Support for Families of Children/ Young People with Disabilities

Embrace offer support to any families with disabled children and young people within Wigan and Leigh. Embrace understand that a "one size fits all" approach can does not work with families and therefore, we tailor our service to your family’s particular needs. Services are ever changing and knowing who to call and where to go can be a task in itself. This is why Embrace ensures that our fieldworkers are knowledgeable in all disability specific areas of family life.

 

How it works

Embrace will take referrals from anyone our only criteria is that the family request our support. Referrals can be made in a variety of different ways, such as, by phone (01942 233323), via social media, through our website (www.embracewiganandleigh.org.uk) and in person at our office. Once we have received your referral, our team will identify which team member's skills best match your particular needs.

We will then allocate a fieldworker accordingly. Once allocated to a fieldworker we aim to ensure this person continues to work with you. When your allocated fieldworker receives your case they will make contact with you to gather further information and arrange a visit. Our visits take place in the place of your choosing, either at your home, at our office or a neutral location of your choosing. On our initial visit our fieldworkers will come and listen to your story, listening to your concerns and what support you require.

After listening to what you have to say the fieldworker will work in partnership with you to create a family centred plan of action. From this point no two cases are the same we will then work alongside you to resolve you're identified needs.
